SCN Program
Specialized Clinical Nutrition (SCN) for eligible Medicaid members
Foodoor partners with authorized New York care programs to deliver medically tailored meals and grocery support to households managing chronic conditions, recovering from a medical event, or living with limited access to nutritious food. The program is fully covered for qualifying Medicaid members — there is nothing to pay out of pocket.

Who qualifies for the program
Eligibility follows standardized New York State screening criteria. Most participants are referred by a care manager, but you can also start the process yourself — we will help verify the rest.
Core requirements
- Active New York State Medicaid coverage
- Resident of Staten Island, the Bronx, or a participating Brooklyn ZIP code
- Enrolled in (or eligible for) an authorized Medicaid care program such as Health Home, HARP, or a Managed Long-Term Care plan
- Demonstrated nutrition-related need — referral or self-attested
Plus at least one of the following
- Chronic medical condition (for example diabetes, heart disease, kidney disease, cancer treatment, HIV)
- Serious Mental Illness (SMI) or Substance Use Disorder diagnosis
- Currently pregnant or in the postpartum period
- Child under 18 living in the household with a qualifying chronic condition
- Recent hospital discharge or recovery from a major medical event

Fill out the online form with your Medicaid CIN, address and household details. It takes about 5 minutes.

Our intake team verifies your information against state program criteria and coordinates with your care manager when needed.

Based on household size, dietary restrictions and clinical needs, we build a weekly meal and grocery plan tailored to you.

First delivery in 1–2 weeks
Once approved, deliveries start on a recurring weekly schedule. We stay in touch to adjust your plan as needs change.
Common questions
- How much does the program cost?
- For eligible Medicaid members, the SCN Program is fully covered. You will never receive a bill, and there are no co-pays, delivery fees, or hidden charges.
- Do I need a doctor's referral to apply?
- A referral helps, but it is not required to start. You can submit the application on your own and our intake team will coordinate with your care manager or provider to confirm eligibility.
- What if other people in my household also have Medicaid?
- List them on the application. Household members enrolled in Medicaid can be included in the meal allocation so the program supports the whole family, not just one person.
- Can I get meals that fit cultural or religious dietary preferences?
- Yes. Menus are built to respect cultural and religious traditions — including halal, kosher-friendly and other preferences — alongside medical dietary needs.
- What areas do you deliver to?
- We currently serve households across Staten Island, the Bronx, and a growing list of Brooklyn ZIP codes. The application will confirm whether your address is in our delivery zone.
- How long does it take to start receiving meals?
- Most approved applicants receive their first delivery within one to two weeks of completing the application, depending on eligibility verification and delivery scheduling.
